Clients with any type of Medicare*:

"Advance Beneficiary Notice of Noncoverage" (ABN)- last updated August 2020: Instructions and Form- we will fill this out together at your first session, if applicable.

Clients with any type of Apple Health or WA Medicaid*:

Agreement to Pay for Healthcare Services- we will fill this out together at your first session, if applicable.
